Need to add collaborators when ticket is first created

We are using 1.9.14

I have a need to allow the end user to add collaborators when they first create the ticket.  What we want is to allow the end user to select their supervisor be included in the ticket thread, receiving email notifications as well.

Any suggestions on how to make this possible?  Thanks!
