I noticed when I update a Help Topic on a ticket that has an associated custom form, the form does not get added to the ticket. I am doing this as an Agent. I click on the Edit button while viewing a ticket. Choose a new Help Topic. I expect the custom form to appear automatically at that point, it does not. I know you can manually add the form, but that adds a bunch of extra steps for an Agent to do. I believe the custom forms should be added (or removed) automatically.
