Installation doesnt show ticket form and login form properly

I just installed the latest version of osticket however when trying to create a ticket, I don't see any fields like title, description, etc. And when trying to register a new user, I just get the fields for password / re enter password?

What's happening here?

I have posted in stackoverflow but no answers.
